
	Livingston Summer Community Camp 2010 Hosted by Gesher Camp


	Location: Kushner Hebrew Academy, Livingston Dates: 23rd Aug - 27th Aug 2010 Peanuts (3&4 year olds) 9am-10.30am $90 Kickers (5-8year olds)/Pro Training (9-14 year olds) 9am-12noon $135 Please ensure players have: Cleats, Sneakers, Shinguards, Water, Sunblock, Sweatshirt or Rain Jacket, Optional Snack Please arrive between 8.45 and 9am each morning. Ensure player is picked up PROMPTLY at 12noon. Please ensure you provide the correct and best contact number(s) when you sign up.

	Health History Please download and complete the health record form. This form must be completed prior to attending any soccer camp. Please indicate all known physical and mental conditions. Indicate if the child hasn't been immunized against diptheria, tetanus, poliomyelitis, measles, perussis, mumps and rubella in accordance with New Jersey State Law N.J.A.C. 8:57-4 and Massachusetts State Board of Health Requirements 105 CMR 430.000. Cancellation and Refund Policy If any portion of a camp or program is cancelled due to inclement weather, a make-up will be scheduled for the lost time. If a make up is unable to be scheduled, a pro-rated credit voucher will be issued. Sessions missed through illness will either be made up for the participant or a credit voucher for the pro-rated value issued.A cash refund will NOT be issued without proper legal cause. A refunded registration prior to camp will incur an administration fee of $25.00. Alternatively, a credit voucher for the full amount paid will be issued if desired. Returned checks will incur a $25.00 fee.
